If you took a distribution from your Investment
Plan account during 2012, a 1099-R form(s) will
be mailed to your address of record by Thursday,
January 31, 2013. Please allow two or three weeks
to receive your form. If you don't receive your form
or if you misplace it, you can request a replacement
form by calling Aon Hewitt at 1-866-446-9377,
Option 4, starting February 15, 2013.
